I hereby, for myself, my heirs, executors and administrators, release, waive and discharge American Foundation for Children with AIDS, its bonafide members and any of the organizers, sponsors, and officials of Climb Up So Kids Can Grow Up events, and any of their respective representatives, agents, successors and assigns, for any and all injuries, damages, or death suffered by me in connection with my participation. I hereby assume full responsibility for all risk of injury, damage or death in connection with Climb Up So Kids Can Grow Up events. I hereby consent to any medical or surgical treatment which I may need and which arises in connection with Climb Up So Kids Can Grow Up events; provided, however, nothing contained herein shall be deemed an obligation to provide any such medical or surgical treatment.
